Responsible for identifyingthe needs of children and families and for providing comprehensive casemanagement services in an effort to empower them to achieve and maintainself-sufficiency. Establish mutually respectful partnership with families toenhance the quality of their lives and their communities. EssentialFunctions Recruits eligible children for NFCD's programs to meetenrollment requirements, including children with disabilities. Completes applications, parent interviews, and enrollmentprocess. Provides case management to support families in identifyingneeds and goals, develop family partnership agreements, make referrals asnecessary, and provide follow up. Collaborates with health services staff to ensure compliancewith all child health requirements. Connects families with community resources to meet basic needsin times of crisis. Documents all aspects of the case management process inNFCD's database system. Documents and provides follow up of excessive absenteeismand attendance concerns as required. The attendance policy ensures compliancewith Rilya Wilson Act. Maintains open and positive communication with familiesthrough ongoing contact and interaction. Greets parents at child arrival anddeparture times to the extent possible. Provides information about communityevents to families. Coach, consult, educate, and utilize counseling skills,where appropriate. Advocate for the family and support them in advocating forthemselves. Facilitate Service Delivery Team meetings (SDTs). Conduct home visits. Ability to respond respectfully to the culture, traditions,lifestyles, language and values of each family and community. OtherJob Functions Supports families in coordinating, preparing and activelyparticipating in parent meetings and annual parent activities and communityevents. Prepares, coordinates, and facilitates presentations andtraining for parents and families as required by performance standards and asdeemed necessary through the result of the Family Needs Assessment. Coordinates with center staff and parents in planningfamily-focused, center-based events and father/male involvement activitiesmonthly. Works collaboratively with center staff to promote anddocument family participation, and provide feedback to parents and staffregarding participation. Works in collaboration with the Family and CommunityPartnership Specialist by participating in community activities that supportfamilies, enhance the programs, and increase community awareness. Maintains effective working interaction with coworkers andoutside contacts that will enhance the operation of NFCD's program. Performs other duties as assigned. ManagementLevel & Responsibilities This position has no supervisory or management responsibilities. Knowledge,Skills & Abilities Required Basic level proficiency with Microsoft Office Suite. Make decisions and acts based on family support principles,theories, practices, and code of ethics. Work within the context of a team approach. The ability to communicate effectively verbally and throughwriting with families, staff, consultants, volunteers, and communitystakeholders. Qualifications– Education & Experience Licenses or Certifications Current Infant/Toddler /Young Child First Aid and CPR Education Degree Type: High SchoolGraduate and is enrolled in an Associate or Credential program for familyservices, social work, or related field Major: Required: Yes Work Experience Required: 1 year of related experiences with strong emphasis onrecord-keeping, tracking, and contact with diverse populations. Preferred: 1 year' experienceproviding services to children and families. Levelof Physical Effort Minimalphysical effort typically found in clerical work. Primarily sedentary, mayoccasionally lift and carry light objects. Walking and/or standing as neededand minimal. PhysicalWorking Environment Minimalexposure to hazards typically found in general office environments where thereis rarely to no exposure to injury or accident. Exposure toroutine office noise and equipment. Background Mustbe able to pass a Level 2 background check. Mustbe able to pass a drug screen. Mustbe able to provide references for the previous 5 years work experience. Additional Contingency Employmentwith NFCD is contingent upon an annual appropriation by the Legislature,federal funding, and successful implementation of job requirements, as measuredby performance-based evaluations. In the event the federal, state, and/or localfunds upon which employment is dependent are withdrawn or redirected,employment may be terminated and NFCD shall have no further liability to theemployee beyond that already incurred prior to the termination date.
